Do I need to pick a new plan?
If you live in Santa Clara County and have The ONE Medicare Advantage coverage that is exiting for 2027, plan materials describe an automatic crosswalk to Harmony (031) effective January 1, 2027. You generally do not have to submit a new enrollment for that automatic move - but you should still read your notices and confirm doctors, drugs, and costs for 2027.
AEP: October 15, 2026 - December 7, 2026 (use it if you want to compare other options available in Santa Clara).
This page is educational. It states the published automatic crosswalk destination. It does not recommend switching to a different product.
What is changing for The ONE members in Santa Clara County?
If you live in Santa Clara County and you have The ONE, that plan is leaving for 2027. Your notice says you will be moved automatically to Harmony (031) on January 1, 2027.
You do not need to fill out a new enrollment for that move. If you already have an agent, they do not need to submit anything for it to happen, and they stay your agent.
Still read the letter you got, and check that your doctors, pharmacy, and prescriptions work under Harmony for 2027. Do not rely on this page alone.
What happens to my coverage on January 1, 2027?
| Topic | What to expect |
|---|---|
| Through Dec 31, 2026 | Your current The ONE rules generally still apply |
| Starting Jan 1, 2027 | Automatic enrollment into Harmony (031), if the crosswalk applies to you |
| If you take no action | The crosswalk usually proceeds as described in your notices |
| Doctors & drugs | Re-check for Harmony in Santa Clara County before Jan 1 |
What to check in AEP (even with a crosswalk)
- Confirm the crosswalk on your notice (plan name and plan ID).
- List Santa Clara doctors, specialists, pharmacy, and prescriptions.
- Confirm whether those providers and drugs work with Harmony for 2027.
- If you want a different option than the crosswalk, compare and enroll during AEP (by December 7, 2026 for a January 1, 2027 start).
- Save confirmations and new member materials.

Common questions
Do I lose Medicare?
No. Part A and Part B continue if you keep them. This page is about The ONE exiting and the automatic move described in plan notices.
Do I have to enroll in Harmony myself?
For this exit, notices say members are automatically crosswalked to Harmony (031). Follow the instructions in your mail if anything asks you to respond.
Can I choose something other than the crosswalk?
Often yes, during AEP, if another plan is available in your county and you enroll by the deadline. This page does not recommend a specific alternative.
Is this a recommendation to buy Harmony?
No. Naming Harmony here only reflects the published automatic crosswalk destination in exit notices. Confirm details on your materials.
