Short answer: In 2026, most people with Medicare pay $0 for Part A and $202.90 a month for Part B. What you pay beyond that depends on whether you have Original Medicare or a Medicare Advantage plan, what drug coverage you have, and whether you buy a Medigap policy. Costs add up for many people. KFF reports that 1 in 4 beneficiaries had income below $24,600 per person in 2024, and that out-of-pocket spending on premiums and cost sharing equaled about 36% of the average beneficiary’s Social Security income in 2023.
This guide walks through the 2026 numbers. It builds on KFF’s October 9, 2026 brief, “How Much Do People with Medicare Pay for Coverage?”, and adds year-over-year changes, Social Security details, and help programs. All amounts are for plan year 2026. The 2027 amounts will be announced later.
What are the main Medicare costs in 2026?
More than 70 million people have Medicare. Here are the headline 2026 amounts from CMS and Medicare.gov, with the change from 2025.

How much is the Part A and Part B premium in 2026?
About 99% of people with Medicare pay no Part A premium, because they or a spouse worked and paid Medicare taxes long enough. People who didn’t can buy Part A for $311 a month with 30–39 quarters of work, or $565 a month with fewer than 30.
The standard Part B premium is $202.90 a month. People with higher incomes pay more through an income-related monthly adjustment amount, called IRMAA. It starts when income is above $109,000 for an individual or $218,000 for a joint filer.
| 2026 premium | Standard | With IRMAA |
|---|---|---|
| Part B (monthly) | $202.90 | $284.10 to $689.90 |
| Part D IRMAA (added to plan premium) | $0 | $14.50 to $91.00 |
People who get Part B only to cover immunosuppressive drugs after a kidney transplant pay $121.60 a month in 2026.
If you sign up late, you may owe a lasting penalty. Part B and Part D both have late enrollment penalties. For Part D, the penalty is based on the national base beneficiary premium, which is $38.99 in 2026. People with limited income and savings may get help with the Part B premium through a Medicare Savings Program, covered below.
What does cost sharing look like in Original Medicare?
Original Medicare uses deductibles and coinsurance. For a hospital stay, Part A has a $1,736 deductible for each benefit period. Days 1–60 have no added daily charge after that. Days 61–90 cost $434 a day, and lifetime reserve days cost $868 a day. In a skilled nursing facility, days 21–100 cost $217 a day.
Part B covers doctor visits and outpatient care. You pay the first $283 each year. After that, you usually pay 20% of the Medicare-approved amount.
The most important thing to know is this: Original Medicare has no yearly out-of-pocket limit. There’s no cap on what that 20% can add up to.
Many people add a Medigap (Medicare Supplement) policy to help pay those gaps. It’s optional, and it has its own premium. KFF found the average Medigap premium among current policyholders was $217 a month in 2023.
What do Medicare Advantage plans cost?
With a Medicare Advantage plan, you still pay your Part B premium. Many plans charge nothing extra on top of that. According to KFF, 75% of Medicare Advantage enrollees with drug coverage pay no additional plan premium, and the average enrollment-weighted premium for these plans is $15 a month. KFF also reports 31% are in plans that reduce their Part B premium, often by less than $10.
Copays and coinsurance vary from plan to plan. Unlike Original Medicare, every Medicare Advantage plan must have a yearly out-of-pocket limit.
| Medicare Advantage out-of-pocket limits, 2026 | Amount |
|---|---|
| Maximum allowed, in-network services | $9,250 |
| Maximum allowed, all covered services (in- and out-of-network) | $13,900 |
| Average in-network limit for enrollees (KFF) | $5,421 |
| Average combined limit in PPOs (KFF) | $9,825 |
There are tradeoffs. Most plans use provider networks, so seeing doctors outside the network can cost more or may not be covered. Many plans also require prior authorization before certain services. It helps to check your doctors and medicines before you choose.
What does Part D prescription coverage cost in 2026?
Part D premiums vary by plan. KFF reports the average stand-alone drug plan (PDP) premium is $36 a month. In Medicare Advantage, the drug portion of the average premium is $8, and 75% of enrollees pay $0 for it.
The standard Part D deductible can be no more than $615 in 2026. KFF reports the average deductible is $544 in PDPs and $371 in Medicare Advantage drug plans. After the deductible, the standard design has you pay 25% until you reach $2,100 in out-of-pocket costs. Then you pay $0 for covered drugs for the rest of the year.
That $2,100 cap started at $2,000 in 2025 under the Inflation Reduction Act and is adjusted each year. It includes your deductible and certain payments made on your behalf, such as Extra Help. Drugs not on your plan’s formulary generally don’t count toward it.
| Median copays, 2026 (KFF) | Stand-alone PDP | Medicare Advantage drug plan |
|---|---|---|
| Generic drugs | $3 | $5 |
| Preferred brand drugs | $45 | $47 |
If big drug costs come early in the year, the Medicare Prescription Payment Plan lets you spread your out-of-pocket drug costs into monthly payments over the calendar year. It doesn’t lower the total, but it can smooth it out.
What changed from 2025 to 2026?
Most costs went up. The Part B premium rose $17.90, from $185 to $202.90. The Part B deductible rose $26, to $283. The Part A hospital deductible rose $60, to $1,736, and the skilled nursing daily amount rose $7.50, to $217. The Part D out-of-pocket cap rose $100, to $2,100.
Social Security benefits rose too. The Social Security Administration set a 2.8% cost-of-living adjustment (COLA) for 2026.
If your Part B premium is taken out of your Social Security check, a “hold harmless” rule can help. It keeps your net check from going down from one year to the next because of a Part B premium increase. It often protects people whose COLA dollar increase is smaller than the Part B premium increase. It doesn’t apply to people who pay IRMAA, people new to Medicare, or some others. Because the COLA covers the $17.90 increase for most people, few are expected to be affected in 2026.
Where can people get help paying Medicare costs?
Extra Help lowers Part D premiums, deductibles, and copays. For 2026, you may qualify with:
| Extra Help limits, 2026 | Individual | Married couple |
|---|---|---|
| Yearly income below | $23,940 | $32,460 |
| Resources below | $18,090 | $36,100 |
Income limits are higher in Alaska and Hawaii. You can apply through Social Security at SSA.gov/extrahelp.
Medicare Savings Programs, such as QMB and SLMB, are run by state Medicaid programs. They can pay the Part B premium, and some also cover deductibles and coinsurance. Limits differ by state, so check with your state Medicaid office. People who qualify for a Medicare Savings Program, Medicaid, or SSI automatically qualify for Extra Help.
What does this mean in practice?
These examples use 2026 amounts. They’re for illustration only, not advice.
Example 1: The baseline. Someone with premium-free Part A who pays the standard Part B premium pays $202.90 a month, or $2,434.80 for the year, before any deductibles, drug plan, or Medigap premium.
Example 2: A long hospital stay with Original Medicare only. Someone without Medigap or other coverage is in the hospital for 70 days in one benefit period. They pay the $1,736 deductible plus 10 days at $434 ($4,340), for $6,076 in Part A costs. Doctor bills under Part B add 20% on top, with no yearly cap.
