Short answer: Not through a SCAN HMO plan, starting in 2027. SCAN Health Plan is leaving Hoag’s Medicare Advantage HMO network when the contract ends December 31, 2026. From January 1, 2027, Hoag doctors and facilities are out of network for SCAN Classic, SCAN Balance, SCAN Heart First, and SCAN Plus HMO members, except for emergency care. Hoag says it takes Original Medicare and some Anthem, Alignment Health, and UnitedHealthcare plans. Check each doctor with each plan before you enroll.
This is a general guide based on what Hoag has published. It doesn’t compare or recommend any plan, benefit, or cost.

Hoag lists these as affected, among others: Hoag Hospital Newport Beach, Hoag Hospital Irvine, Hoag Orthopedic Institute, Hoag Medical Group, Hoag Physician Partners, and Hoag Family Cancer Institute.
Which SCAN plans are affected?
Hoag’s Medicare page lists SCAN Classic (HMO), SCAN Balance (HMO SNP), SCAN Heart First (HMO SNP), and SCAN Plus (HMO) with the note “Contract Expires 12/31/2026.” Hoag’s FAQ describes the change as applying to SCAN Medicare Advantage HMO members. If your SCAN plan isn’t on that list, ask SCAN directly whether Hoag is in your 2027 network.
Can I still see my Hoag doctor for the rest of 2026?
Yes. Hoag says it stays in network with SCAN through December 31, 2026, so you can keep seeing Hoag doctors and using covered Hoag services through the end of the year.
Can I still go to the Hoag ER in 2027 with SCAN?
Yes. Hoag says the change doesn’t affect emergency treatment at Hoag hospitals, and that emergency care is always considered in network. In an emergency, go to the nearest emergency department.
I’m in the middle of treatment with a Hoag doctor. What happens?
You may be able to keep seeing that provider for a set period under continuity of care. Hoag says SCAN decides who qualifies. To ask, call SCAN using the number on the back of your member ID card. Don’t assume you qualify until SCAN confirms it.
Which Medicare plans does Hoag take for 2027?
As of October 14, 2026, Hoag’s SCAN FAQ says it participates in Original Medicare and these Medicare Advantage plans:
- Anthem: Anthem Select (HMO-POS), Anthem Medicare Advantage (HMO-POS), Anthem Prime (HMO-POS)
- Alignment Health: Alignment Health Platinum Preferred 004, Alignment Health Honor Plus 060
- UnitedHealthcare: AARP Medicare Advantage from UHC CA-46
Hoag’s Medicare page also lists “most” Medicare Supplement (Medigap) plans alongside Original Medicare.
Some things to keep in mind:
- A hospital being in network doesn’t mean every Hoag doctor is. Check each doctor, and the medical group, with the plan.
- Availability depends on where you live. In CMS’s 2027 plan landscape file (September 2026), AARP Medicare Advantage from UHC CA-46 (HMO-POS) is listed in Orange County only. Check plans by your ZIP code.
- This list is Hoag’s, and it can change. It isn’t a list of every plan that may include some Hoag providers, and it doesn’t say anything about other UnitedHealthcare plans.
Do I get a special enrollment period because Hoag left SCAN?
Not automatically. Medicare rules include a special election period for people affected by a significant change in a plan’s provider network (42 CFR 422.62(b)(23)). It applies only when CMS treats the change as significant, and in that case the plan sends eligible members a notice. If you get one, it explains your window and options. Don’t count on it unless you’re notified.
What are my options if I want to keep Hoag?
- Use Annual Enrollment (October 15 to December 7, 2026). You can switch Medicare Advantage plans or go back to Original Medicare. Changes take effect January 1, 2027.
- Check your doctors first. Ask each Hoag doctor’s office whether they take the exact plan name for 2027, and confirm with the plan. Our guide on how to check if your doctor is in a Medicare Advantage network for 2027 walks through it step by step.
- Fallback: the Medicare Advantage Open Enrollment Period (January 1 to March 31, 2027). If you’re in a Medicare Advantage plan on January 1, you can make one change during the first three months of the year: switch to another Medicare Advantage plan or go back to Original Medicare (42 CFR 422.62(a)(3)(i)).
- If you stay with SCAN, expect a new PCP assignment and ask SCAN about continuity of care if you’re in treatment.
